Fort Campbell, Kentucky – Blanchfield Army Community Hospital (BACH) will be hosting a walk-in school and sports physical clinic, Saturday, July 16 and 23, 2022 from 8:00 a.m. to noon at the Byrd Army Family Medical Home, 7973 Thunder Boulevard, Fort Campbell.
These appointment dates can be used for children in the care of a primary care manager at BACH or an off-post PCM in the TRICARE network.
School and/or sports physical exams are required for:
- Any child entering Christian County or Clarksville-Montgomery County schools for the first time
- Any teenager who enrolls in Grade 6 at Christian County or Fort Campbell schools
- Any teenager entering 7th grade in Tennessee
- Any student participating in sports organized by the school
- Any youth participating in Fort Campbell’s organized sports programs
“School physical exams are an important periodic wellness check required by most schools at critical stages of a child’s development and prior to participation in competitive sports. These visits are a great opportunity to assess a child’s physical and emotional health, educate them about health and social issues, and ensure they are on the right developmental path,” said said Lt. Domenick Nardi, BACH’s Chief of Primary Care and board certified. pediatrician. “The older kids get, the less likely they are to go to the doctor unless they get sick, so this is a great opportunity to make sure there aren’t any pressing issues.”
Parents are encouraged to download the appropriate school or sports forms required by the child’s school district and complete the forms before arriving for the physical appointment. Children should wear shorts and a short-sleeved shirt to facilitate the examination.

Children who have undergone school or sports training in one of BACH’s medical homes within the last 12 months and who have not undergone any medical changes, may not need a new examination. Parents can simply contact their child’s care team to have the required school forms completed. Administrative formalities can take up to three working days.
School and sports physicals are also available by appointment on weekdays for families assigned to a PCM at BACH, including Screaming Eagle, Young Eagle, Air Assault and Byrd Medical Homes by calling the appointment line at 270.798. 4677 or 931.431.4677, or by emailing your care team using TOL secure email.
When scheduling an appointment, please let the team know if the child will be attending a school in Kentucky, Tennessee, or another state during the 2022-23 school year.
Parents of patients with more complex or complicated conditions, or who may need more time, should schedule a separate visit with their PCM during a normal appointment time. Care teams will not treat any acute or chronic medical conditions during drop-in school and sports physical clinic days.
